Waiting List Form

In order to be eligible to be offered a place at C&K Ashgrove West Preschool:

The waiting list form must be completed and submitted for each child; AND
Pay the $10 waiting list fee for each child.

To submit your waiting list form, you may either:
Download the pdf version of the waiting list form and post it with payment of the waiting list fee;
OR
Complete the online form below and pay your waiting list fee.

Direct Deposit can be paid here.
Cheques are accepted an can be made payable to “Ashgrove West Preschool Centre”.
Credit card by calling the office on (07) 3366 1212.
